Valley Health Team accepts Medi-Cal, Medicare, most private insurances and self-payment. Discounts are offered to our self-pay patients if payment is made at the time of service.
Sliding Fee Scale Program
Valley Health Team (VHT) has a discounted Sliding Fee Scale Program for patients based on income and family size. No patient will be denied services due to an inability to pay. VHT can also help you determine if you qualify for any third-party coverage. Please ask the front office staff for assistance.

Valley Health Team is a Certified Enrollment Entity and can help you with the application process as well as answer any questions you may have.
Sliding Fee Scale Program
Valley Health Team, Inc. is a Federally Qualified Health Center (FQHC). We have staff available to assist patients in determining if they are eligible for a variety of health benefits coverage options. In many cases, FQHC staff can assist qualifying patients with enrollment.
FQHCs offer discounted medical services to qualified patients. Eligibility is based solely on income and family size. Please bring ONE of the following to the health center to determine if you are eligible:
- Current tax return
- Most current pay check stubs
- Unemployment award letter or 1 unemployment check stub
- Letter from employer on official business letterhead that states salary paid and how often
If you do not have one of these documents, there are other forms of documentation accepted. Please see a Health Center representative for assistance.
